Part 1 - Early Planning

Part 1 should be filled out on a continuing basis during the early stages of your engagement and planning. Please list and review the businesses where you shopped for and/or purchased your wedding gown and bride's lingerie.

2. Bridesmaids, Mother of Bride and Flower Girl Dresses.

Please list and rate the businesses where you shopped for and/or purchased Bridesmaids, Mother of Bride and Flower Girl Dresses.

3. Reception Site.

Please list and review the Reception facilities that you visited and considered during your search for your reception site.

4. Ceremony Location (Chapel) and Officiant.

(Note: Churches of a religious denomination and their ministers are not reviewed here).Please list and review only the commercially available locations (Chapels) that you have considered and/or selected as a venue for hosting your marriage ceremony. You may also review any marriage officiant who is available for hire. Note: Churches of a religious denomination and their ministers are not reviewed here.

5. Bridal Shows.

Please list and review the bridal shows that you have attended.

6. Gift Registries.

Please list and review the Wedding Gift Registries where you have enrolled.

7. Engagement Rings, Wedding Rings.

Please list and review the businesses where you shopped for and/or purchased your engagement ring and wedding rings.

Part 2 - General Wedding Planning

Part 2 should be filled out on a continuing basis during the early stages of your engagement and planning. You may stop back at any time to complete unanswered questions or to enter reviews of additional merchants for a particular question. You may also enter additional comments to your original entries.

1. Culinary (Wedding Cakes, Catering, Confections).

Please list and review the wedding cake businesses, caterers, chocolate fountain rentals, confectionary, candy and dessert businesses that you considered during your wedding planning.

2. Decorations & Rentals.

Please list and review the rental companies, balloon and decoration, dove release, ice carving and special lighting companies that you visited and considered during your wedding planning.

3. Wedding Services.

Please list and review companies or individuals that you considered for party planners, event planners and consultants and party suppliers that you visited and considered during your wedding planning.

4. Health and Beauty.

Please list and review any business that you used or considered for fitness and weight loss, makeup, hair and spa services, cosmetic dentistry, and cosmetic or laser surgery.

5. Invitations - Favors - Gifts.

Please list and review your suppliers for invitations, stationary, reception favors, gift baskets and bridal party.

6. Music & Entertainment.

Please list and review businesses that you considered for live music, DJ services, dance lessons, and vocals.

7. Photography.

Please list and review the photographers that you consulted with and considered during your wedding planning.

8. Rehearsal Dinners & Parties.

Please list and review the rehearsal dinner and bachelor/bachelorette party locations that you considered during your wedding planning.

9. Transportation.

Please list and review horse drawn carriage companies, limousine companies and car rental companies that you considered during your wedding planning.

10. Tuxedos and Men's Formal Wear.

Please list and review the Men's formal Wear companies that you consulted with and considered during your wedding planning.

11. Video and Cinema Professionals.

Please list and review the videographers that you consulted with and considered during your wedding planning.

12. Flowers.

Please list and review the florists and floral design companies that you visited and considered during your wedding planning.

Part 3 - After Wedding Evaluation

Part 3 may be started only after your wedding has taken place. In this section you will review only the businesses that you actually used for your wedding. In many cases you may have previously reviewed a particular business in part 1 or part 2, however, you are now giving your final evaluation based upon service performed or product delivered. You may stop back at any time to complete unanswered questions. You may also enter additional comments to your original entries.

1. Reception Site.

Please list and review the Reception Location that you actually used for your wedding.

2. Ceremony Location (Chapel) and Officiant.

Please review the hotel(s) that you used for your out of town guests.

8. Music & Entertainment.

Please list and review the live band, strings or ensemble, DJ service, dance instructor, and vocal performer that you used for your wedding.

9. Rehearsal Dinners & Parties.

Please list and review the rehearsal dinner and bachelor/bachelorette party locations that you used for your wedding.

10. Transportation.

Please list and review horse drawn carriage company, limousine company and car rental company that you used for your wedding.

Please list and review the Dry Cleaner who cleaned and preserved your wedding dress and the business that Freeze Dried, Pressed or Preserved your floral bouquet.

15. Your Future.

Please list and review any businesses that you used or depended upon for services such as interior design and decor, furniture purchases, new home or apartment, mortgage broker, insurance, financial planning, legal services.

16. Photography.

Please list and review the photographer that you used for your wedding.
